Flexible capture
Use recording, dictation, or typing based on the patient, setting, and consent available for that session.
Documentation · AI SOAP notes
After a visit, record, dictate, or type what happened. SoapBox turns those details into a SOAP draft, and you decide what stays in the chart.
From session to signed note
Use the capture method that makes sense for that session.
Record, dictate, or type the session details. Chunked upload and retry-aware processing support longer recordings and interrupted connections.
SoapBox uses the session context, goals, cueing, and progress data to draft a structured SOAP note for the selected discipline.
Edit any text or regenerate one section without replacing the rest. The clinician remains responsible for the final documentation.
Built for real documentation work
Use recording, dictation, or typing based on the patient, setting, and consent available for that session.
SLP, OT, and PT workflows use different documentation context instead of forcing every clinician into one generic note style.
Revise the Subjective, Objective, Assessment, or Plan independently while preserving the sections you have already reviewed.
Export supported documentation as PDF or DOCX, or use the Chrome extension with compatible web-based EMR workflows.
On supported mobile builds, encrypted recording segments can remain on the device through a network interruption and upload before processing begins.

Ask us a question →Yes. SoapBox supports SLP, OT, and PT documentation with discipline-aware inputs and drafting context.
Yes. Each SOAP section can be edited directly or regenerated independently before the note is signed.
Long recordings use chunked uploads and retry-aware processing. An interruption may delay completion, but SoapBox keeps the workflow recoverable instead of treating partial processing as a finished note.
On supported mobile builds, recording segments are encrypted on the device and can continue uploading after a connection interruption. Processing starts after the complete recording is received and validated.
SoapBox does not use recordings or notes to train its own general-purpose AI models. They are processed to provide the requested service. Production PHI is sent only through approved workflows to service providers covered by the contractual safeguards required for that processing, including BAAs where required.
